Did this ever get 'issued'?

I've never seen one, only the 'demo'

Not unless you count the later 70's legitimate MGM special products  issue.

 

I got one demo from Bostocks stall at Bradford market in the early 70s which was unusual as all the other 1000s of Verve records they had were  issues. If there were issues I'm sure they would have had them.
