Just Came across this one the other day & thought I'd offer it here. Have yet to find anyone that knows much about this release & have only seen three others on this label (Chicago or Rockford, IL label??). It's in decent condition..(light marks/light noise), but you can check the soundclips and hear for yourself. The B-side has the instrumental version (posted full version of a-side & clip of the instrumental).
